Amenity Use Agreement

I, the undersigned, in consideration for Oak Tree Farm Clubhouse Homeowners Association (“OTF”) granting exclusive use of the OTF amenities to include community pool, recreational courts, and clubhouse (collectively referred to as "amenities"), agree to the following:

1. To always maintain possession of our household’s activity fob(s) and to not allow use of our fob(s) by any person other than members of our immediate household or a responsible person chaperoning our underage children.

2. The OTF amenities are available for our household, and we can host guests in these spaces if a member of our household is present.

3. My household is responsible for the actions and behavior of all people present at the amenities at our request or invitation, and we are personally responsible for causing all such people to comply with the amenity rules.

4. OTF will provide one amenity fob to each household at no charge. A second fob, or a replacement for lost or damaged fobs, may be acquired for $25.

5. I have carefully read and will abide by the posted OTF amenity rules and this Amenity Use Agreement. I understand that any violation of the aforementioned rules could result in suspension of pool, court, and clubhouse privileges.

6. I accept total responsibility for “swim at your own risk” times and I hold harmless OTF, the OTF Board of Directors, and event volunteers, from all claims resulting from injuries or other damages arising out of use of the OTF pool.

7. I understand use of all OTF facilities is "use at your own risk" at all times. I hold harmless OTF, the OTF Board of Directors, and event volunteers from all claims resulting from injuries or other damages arising out of use of the OTF recreational courts + clubhouse.

NOTICE: By signing below, you are certifying that the above information is accurate, and you are agreeing to the rules outlined in the amenities agreement.
